Clearance to Participate in Sports
In order to participate in sports, including practices, all students must be registered for the sport and have a current physical examination on file in the Athletic Department. To ensure clearance for participation, registration should be completed and physicals should be turned into the Athletic Department no later than 2 school days prior to the first day of official practice.

On April 23, 2024, the CHSAA Legislative Council approved to install the updated Preparticipation Eligibility Form (PPE) as the required and only form that will be accepted by schools starting 2024-2025 (pilot year) with full integration 2025-2026. Click HERE to access the physical form. Upload your physical into family registration account after appointment as follows:
1. Login to your family registration account.
2. Toggle Add under the Physical Date column.
3. Follow the prompts to upload the new physical form.
4. Don't forget to Save before exiting.
Physicals are valid for one year (365 days) from date of the last exam.
